The service works in collaboration with adults and older people that are resident in Islington, alongside their relatives/carers, to promote health and maximise independence in mobility, communication, work, leisure and everyday activities.
 
The service consists of two teams:
  • Stroke and Neuro
  • General Rehab and Falls

 

Both teams consist of occupational therapy, physiotherapy, speech and language therapy and therapy assistants.
 
Referral pathway is through the integrated health and care front door.
 
Service operates Mon – Fri 9 – 5pm

GP referrals:
EMIS - complete the Islington Integrated Health Referral Form – District nursing and Community Therapists and send to rapidaccess@islington.gov.uk
 
Where to find the form:
  • Islington: ISL Global Documents > Community Services
 
All other referrers:
Complete the Islington integrated Referral for District Nursing & Community Therapist form.

Please note, we do not accept self-referrals unless the patient has already been under the care of the Stroke and Neuro, or speech and language team, and they can self-refer back into the service within 6 months of their discharge date using the contact details on their discharge form.
 
 

Eligibility Criteria

Stroke/Neuro

  • residents of Islington aged 18+ with therapy needs because of a neurological condition including: stroke, Parkinson’s disease, multiple sclerosis, brain injury, motor neurone disease, cerebral palsy, brain tumours and other neurological problems
  • early supported discharge from local hospitals after acute stroke.

 

General Rehab and Falls

  • Islington resident
  • has rehab needs/goals
  • willing to participate in rehab in their community
 
Exclusion criteria for both:
  • referrals for equipment or wheelchair provision
