How to get referred

Only doctors from the referring hospital can refer (unless the child has a self referral care plan in place). Children must have an Islington GP. Visits required more than 3 times per day are not suitable. Referrals must be made via email. Please call the hospital at home hot phone to check capacity before referring. Referrals received after 5 pm May not be seen until the following day. All medications need to be ordered and supplied by the referring physician prior to transfer home.
