Children's Community Nursing Services

The Children’s community nursing team consist of highly trained and experienced paediatric nurses. The nurses visit children at home to deliver nursing care to children with both long term and acute conditions. The nurses work closely with allied professionals such as dieticians, community paediatricians, psychologists, clinical nurse specialists, physiotherapists, occupational therapists, school nurses and social workers.
The Children's Community Nursing Team is made up of highly skilled and experienced paediatric nurses who provide specialist nursing care for children and young people in community settings, including their homes, schools and community health centres.
 
The service also includes:
  • Children and Young People’s Hospital at Home (H@H) covering Camden, Islington and South Barnet
  • Paediatric Primary Care (PPC) Team covering Islington ONLY
  • North Central London (NCL) Paediatric Bowel and Bladder Service, covering Camden, Islington, Haringey, and Barnet postcodes.
The team works closely with children, young people, families, and other healthcare professionals to provide high-quality, family-centred care. Our aim is to prevent unnecessary hospital admissions, support early discharge from hospital, and reduce the length of hospital stays for children with both short-term and long-term health needs.
 
Eligibility
Children referred to the service must:
  • Have an identified nursing need at the time of referral.
  • Live within the London Borough of Islington (except for the NCL Paediatric Bowel and Bladder Service, which also covers Camden, Haringey, and Barnet).
The service provides care for:
  • Children and young people aged 0–17 years.
  • Up to 18 years with complex health needs going through transition.
 
Specialist Services
The Children's Community Nursing Team provides specialist care and support in the following areas:
  • Wound care
  • Feeding support, including nasogastric (NG) tube feeding and gastrostomy care.
  • Oxygen monitoring.
  • Non-invasive ventilation care.
