Anticoagulation and stroke prevention services

The Anticoagulant clinic is for patients who need to be monitored because they take warfarin or another anticoagulant drug. You will need to attend this clinic because your GP or another specialist doctor will refer you to the clinic following the start of anticoagulant drug treatment with warfarin. Warfarin is used to treat heart problems or a blood clot in a blood vessel. For more infomation on Warfarin, please read the sections on the right hand side.

First of all you will need to have a blood test in the Haematology department on level 5 of Outpatients. Afterwards, you should come down to the Anticoagulant clinic on level 3 where you will be given a pink form with your results, which we will be discussed with you. The anticoagulant specialist nurse or pharmacist will check your International Normalised Ratio (INR) levels following your blood test to make sure that your dose of warfarin is correct for you. This is important because your needs can vary over a period of time.

Our hospital anticoagulant and stroke prevention service is led by clinical nurse specialist Soi-Soong Choi. She heads a dedicated team who work at the hospital and also in the community. In conjunction with North Middlesex Hospital, we also oversee the quality of the delivery of anticoagulant and stroke prevention services at the following sites:

The service provides:
  • Assessment and counselling sessions
  • Advice and information
  • Short and long term follow-up care
  • Health education and promotion
  • Ongoing support, primarily via telephone, for patients, relatives and carers
  • Information and advice to other healthcare professionals
  • International Normalised Ratio (INR) testing.
