The intermediate diabetes service provides diabetes management to patients living in Haringey and Islington.

The team includes diabetes specialist nurses, diabetes specialist dietitians, diabetes psychologist (Islington only) and support from a diabetes consultant.

Diabetes clinics take place at a variety of locations throughout Haringey and Islington depending on where the patient lives. Domiciliary visits are also provided.

Group education and self-management is a key component of the service. Islington runs 'DESMOND' (diabetes education and self-management for ongoing and newly diagnosed) for people with type 2 diabetes. Haringey runs conversation maps for people with type 2 diabetes. 

A seven week self-management programme is available to patients, which aims to help participants to build skills, knowledge and the self-belief needed to effectively manage their diabetes.
